What should I look for at an accessible foundation-drain outlet?
What can a routine look at an accessible foundation-drain outlet actually tell you? I’m curious what’s normal to see there after dry weather versus after rain, and whether loose leaves or sediment at the opening can be cleared without risking damage or disturbing the soil around it.
What signs suggest the obstruction or damage is farther inside the drain and worth having a drainage professional inspect?

After dry weather, little or no flow may be entirely normal; discharge after rain shows water is reaching the outlet, but neither observation alone proves the whole drain is clear. You can gently lift away loose debris right at the opening, without digging, pushing tools inside, or shifting soil. Persistent poor discharge after rain, recurring sediment from inside, or visible damage are reasons to have a drainage professional inspect farther along the line.
