Turned out the issue was actually sediment buildup in the pipes. Older houses especially can have this problem—little bits of rust or minerals break loose and clog things up randomly, causing pressure fluctuations.
Ended up flushing the pipes thoroughly and installing a sediment filter right after the main shut-off valve. Honestly, haven't had an issue since. Might be worth checking if your pipes are older or if you're on well water. Regulators usually last a good while unless you get really unlucky with a dud (which does happen occasionally). But before spending more cash on another regulator, I'd definitely look into sediment or debris issues first. Could save you some headaches down the road...
