What does an after-hours water-heater call-out fee actually cover?

If a water heater issue comes up at night, what should I clarify before agreeing to an emergency visit? I’d want to know whether the quoted call-out fee covers dispatch and diagnosis only, or whether it’s credited toward repair labor if work is approved.
It also seems worth separating the after-hours labor rate from parts and any additional trip charge. If the heater can be safely shut off and there’s no immediate safety concern, what findings would make you recommend repairing it overnight rather than scheduling the work during regular hours?
